What you send us: If you use a contact form or email link, we may receive your name, email address, phone number, and message. Please do not include health or medical information in contact messages — this website is not a HIPAA-covered channel.

By booking through SimplePractice, you agree to its Terms of Service and Privacy Policy, as well as our practice's intake documents and policies. Keys Pelvic Floor Wellness is not responsible for the availability or performance of the SimplePractice platform.
